Required 2020 Forms for All Students
All Millcreek Cyber, 100% Virtual, and A/B Hybrid students require the following forms be completed by a parent or guardian, signed, and returned to keep on file for the 2020-2021 school year.

- Media Release Consent
How to submit 2020 Required Forms:
1. Click the Link Below to access the 2020 Required Documents File.
HIGHLY RECOMMENDED to complete the documents on a Chromebook, laptop, or desktop if possible.
If using a mobile device is required, download the file below to the mobile device and then open to sign using a mobile app such as Free Adobe Fill & Sign.2020 Required Documents to Download
2. Review the instructions on page one for signing and submitting the forms.
3. Please review, sign, save and submit your forms to the Google 2020 Upload Form (link below) using your MTSD student email account
EACH form should be completed for EACH child in the District. For example, if you have three children attending MTSD schools, 3 of each form should be submitted.
