Changing Your Address
Have you or your family recently moved?
Submit the documents and copies of requested items below to update your child’s permanent school record and household information.
Complete your address change by using the fillable form below. You may return with proofs of address via email or in-person.
- Change of Address Form
- Proof of ID: Valid PA Driver's License, PA ID Card, or Address Update Card
- Proof of Public Utility: Gas, electric or water bill (or email confirmation of Penelec service start-up).
- Proof of Residence (if owning): Property or School Tax Bill, Deed OR Closing Disclosure. Note: Sale of Agreement accepted until final closing.
- Proof of Residence (if renting): Current Lease, signed and dated by landlord/property manager and parent who also must be listed as a tenant.

In-person at the Millcreek Education Center, located at 3740 W 26th St. Erie, PA 16506. Central Registration hours: Monday – Friday 8am-3:00pm.
Busing: First Student Transportation will notify you of the new bus routing via email within five days. Please plan to transport your child to/from school until the new bus routing is confirmed.
