Changing Your Address

Have you or your family recently moved? 

Submit the documents and copies of requested items below to update your child’s permanent school record and household information.

Complete your address change by using the fillable forms below. You may return with proofs of address via email or in-person.

Proofs of Address (must provide three – see below)

  1. Proof of Identification:
  • PA Driver’s License*, US Passport, Military ID or PA State ID                                  

            *Provide additional proof of address IF license is not current.  Must have photo ID.

2.  Proof of Public Utility:

            Gas, Electric or Water Bill                                                     

3.  Proof of Residence:

            Rentals - current Lease including signatures of landlord and tenant.                      

            Homeowners – School or Property Tax Bill or Deed or Closing Disclosure 

             Note: Sale of Agreement accepted until final closing          

Submit Forms and Documents

Submit online to our office email:  registrationdocs@mtsd.org or in-person at the Millcreek Education Center, located at 3740 W 26th St. Erie, PA  16506.  Business hours:  Monday – Thursday 8am-3:30pm or Fridays 8am-3:00pm.

Busing:  Our transportation supervisor will notify you of the new bus routing via email within five days.  Please plan to transport your child to/from school until the new bus routing is confirmed.
