Note: Dates should be entered as MM/DD/YYYY and phone numbers as xxx-xxx-xxxx.
Parents must input the following data into the application:
- Household Information - home address or mailing address
- Personal Information - work and cell phone numbers, email addresses
- Emergency Contact - addresses and phone numbers
Parent must prepare the documents listed below as an electronic version to upload to the application. You cannot save the application without uploading to the required fields, marked with a RED asterisk. These fields are included in the Students Information tab:
- Child's Birth Certificate
- Custody/Court order if applicable
- Immunization records
- Special Education documentation, if applicable
- Complete the Home Language, Parent Registration and Request for Records form
- Proofs of Address: Valid PA Driver's License, PA ID Card, or Address Updated Card; AND Utility bill (Gas, Electric, OR Water); AND Property Tax BIll, Deed OR Closing Disclosure. If renting, a Current Lease (signed and dated)
