Important to note before starting:

For best results, use Google Chrome on a desktop or laptop device. 

Dates should be entered as MM/DD/YYYY and phone numbers as xxx-xxx-xxxx.

Parents must input data or ‘fill in the blanks’  to the online application in each of these sections:

  • Household Information - home address or mailing address
    • Your address is not a guarantee of which school your student will attend. Boundaries alone are not the sole determining factor in student placement.
  • Personal Information - work and cell phone numbers, email addresses
  • Emergency Contact - addresses and phone numbers
  • Student Information - biographical information specific to the student

The following documents must be saved as an electronic version to upload to the application. These fields are marked with a Red asterisk on the Student Information tab:

  • Child's Birth Certificate - biological parent(s)must be listed on this document
  • Custody/Court order or Legal Guardianship order if applicable 
  • Immunization records (most recent)
  • Special Education documentation, if applicable 
  • Proof of ID:  Valid PA Driver's License, PA ID Card, or Address Update Card
  • Proof of Public Utility:  Gas, electric or water bill (or email confirmation of Penelec service start-up).
  • Proof of Residence (if owning):  Property or School Tax Bill, Deed OR Closing Disclosure.  Note: Sale of Agreement accepted until final closing.       
  • Proof of Residence (if renting): Current Lease, signed and dated by landlord/property manager and parent who also must be listed as a tenant.  
  • Are you living with another Millcreek resident who owns a home?  Please call the registration office at 814.835.5312 for specific requirements.
  • Complete these ‘fillable’ forms:
    • Home Language Survey, Parent Registration, Request for Records and Earned Income Tax form

It is the parent/guardian's responsibility to provide the required documents. Missing documents will delay your student's enrollment.

Our online application is also available via our new KIOSK.  Please schedule an appointment by calling our office at 814.835.5300 ext. 2.  Bring your required documents to your appointment; we will scan to the application for you.  We are located at 3740 West 26th St., Erie, PA 16506.

Summer School Registration Information

Summer school is for existing MTSD students who either need a remediation course or would like to do an acceleration course. For more information, see the information sheet below. Please contact Mr. Gilroy at gilroy@mtsd.org with any questions.
