Summer News from our School Nurse

Happy Summer Break! These are a few reminders from the School Nurse:

If your child has any updates to their medical history whether temporary or long term, please update the school nurse by either completing a health history update form or contacting the school nurse via email or by phone. If you need any forms or have concerns, please feel free to contact the school nurse

If your child has a serious health condition (asthma, life-threatening allergies, and seizure disorders) please complete an action plan which can be found on the Health Services page on the school district website.

If you would like your child to have medication in school, please complete the Authorization for Medication form and have it signed by both the parent and the physician. We do have a list of medications that are by standing order, but they will only be administered if a parent provides their consent in writing AND provides their own supply of the medication.

For students entering 6th grade

All 6th grade students are required to have a physical for this school year. It is HIGHLY RECOMMENDED that all students are seen by their private physicians in order to have continuity of care and to receive their state mandated vaccines. Any student that does not have their mandated vaccines will be unable to start 7th grade unless they have religious, medical, or moral exemptions on file. For specific questions regarding your child, please call the school nurse. We do accept physicals that are done up to one year prior to the start of the school year.

For students entering 7th grade

All 7th grade students need to have a dental exam for this school year. We do offer exams at no charge to the parent, although we recommend private exams so your child may have treatment and/or correction as needed. Please turn in all private dental exams before October 1st.

All forms including medication authorization, physical exams, and dental exams are available online on the MTSD Health Services homepage or in the school nurse’s office.

Thank you
